Description

Enroll now in the Spiritual Union of Our Lady of Czestochowa and share in the Holy Masses, Prayers, Novenas, Sacrifices, and Good Works offered by the Franciscan Brothers and Staff of the Black Madonna Shrine.

There are two versions of the Enrollment:

Enrollment A: The first is the size of a greeting card with a full-color picture of Our Lady of Czestochwa on the cover, the name of the enrolled person and the name of the person who enrolled them within. An offering of $10.00 per person/family/enrollment is requested to help support the Black Madonna Shrine.

Enrollment B: The second is a certificate in a folder (approximately 9 x 12) with a full color picture of Our Lady of Czestochowa, the name of the enrolled and the name of the person who enrolled them on the certificate. An offering of $20.00 per person/family/enrollment is requested to help support the Black Madonna Shrine.
